FMLA Health Benefit Continuation Notice

Employer must maintain group health coverage during FMLA leave on same terms as if employee continued working. Must provide notice of obligation to pay employee share of premiums, consequences of non-payment, and conditions for recovering employer-paid premiums if employee does not return.
