GINA Wellness Program Safe Harbor Notice

GINA; 29 CFR § 1635.8(b)(2)

If a wellness program collects genetic information (including family medical history via HRAs), employer must provide a notice that: providing genetic info is voluntary, info will not be used for underwriting, only aggregate data is disclosed to employer, and info is protected by GINA.
